We started Reviving Mind because there are so many out there who simply don't have access to the support that they need.
Reviving Mind brings together small groups of 6-10 people of older adults to work on their chronic conditions, changing mental and physical wellbeing needs through group support with 1 on 1 lifestyle coaching sessions all facilitated by a dual-certified provider matched to you.
The 360º Program is for everyone who's going through the typical challenges that come with aging and life such as chronic conditions, mental health issues and loneliness.
The 360º program can be an added source of support that can provide you with chronic care management for chronic conditions such as (but not limited to) diabetes, chronic pain, hypertension and anxiety.
In an effort to make the program more accessible for everyone, we're now accepting Medicare for all MA residents. Speak with our supervising RN to see if you qualify for coverage.
Get a 1-on-1 assessment meeting with our NP to map out your chronic care management plan and health goals. You'll then get matched to a group and provider that's a perfect match for you.
Your provider who facilitates both your support group and individual Lifestyle Medicine coaching sessions will establish mindful agreements within the group and guide you in building a sense of connection by getting to know your peers through fun group exercises.
As a group, you'll explore each other's experiences, emotions and thoughts. Members discover new ways of thinking and being as a person and group member, with your provider as an encouraging guide. During 1 on 1 sessions, your provider makes sure to check up on you and keep you on track of your chronic care management plan.
This is a time to reflect on the ways you and your group has progressed towards the first stage of the program, looking back at the key lessons of the past weeks. Many of our groups continue on together for another 12 weeks and groups set further goals and an intention for the next stage.
